The STAR Health Center’s Clinical Quality Management Program is comprised of a robust quality management infrastructure, ongoing performance measurement for quality Improvement (QI), and a dynamic timetable of quality Improvement projects. Our Quality Steering Committee meets quarterly, and consists of STAR’s Program Director, Medical Director, Peer Leader/Educator, Quality Lead Physician, Clinic Operations Manager and Director of Quality and Community Outreach.  The STAR Health Center has maintained NCQA Accreditation for a Level III Patient Centered Medical Home since 2013.

STAR Quality Management processes include collecting, analyzing and reporting data regarding patient care, health outcomes and satisfaction. Measures reflect local HIV epidemiology, identified needs of local residents living with HIV/AIDS, and are annually evaluated and compared with state and national HIV/AIDS strategic goals. A strong emphasis is placed on identifying and resolving disparities within patient sub-populations.

Stakeholder Involvement is ensured through facilitation of consumer advisory meetings, patient surveys, monthly breakout groups at all-staff meetings, and input from the Diversity, Equity, and Inclusion Committee.

The Model for Improvement (MFI) quality methodology is used as part of overall clinic activities, using a team approach and addressing underlying systems of care. To facilitate an overall clinic mindset and capacity to conduct drill downs, develop priorities and conduct PDSA cycle tests, key program staff are assigned to participate in QI trainings and projects.
