The STAR Program’s “Back to Brooklyn” (or B2B) Program is a community-based medical home for people who have experienced incarceration. Grounded in patient empowerment, B2B provides immediate access to comprehensive medical care, behavioral health care, and support services to those re-entering the community.
B2B is funded by the NYS Department of Health AIDS Institute’s Corrections Health Initiative and the Mother Cabrini Foundation. The project provides comprehensive care to individuals with chronic health conditions returning to the community from New York state and federal prisons and New York City jails.
Eligibility
Eligibility for B2B services requires that clients: 1) have a chronic condition (e.g., diabetes, high blood pressure, or HIV), and 2) are within 90 days of release from a New York State or City correctional facility.
Services Provided
On the release date, B2B’s Community Linkage Specialist (CLS) will meet the client at the bus/train station (if not logistically possible, within the first 24 hours of release).
The CLS will accompany the client to their first parole appointment and will link them to medical care within one week of returning to the community.
